About Our Handling of Personal Information

As part of its efforts to protect personal information the Company will make its handling of personal information readily accessible to individuals and matters that should be disclosed are as follows. (In compliance with JISQ 15001:2006.)

5. Provision to third parties

Personal information is provided to third parties only with the consent of the individual and when permitted by exceptions under the law. (2) Documents to submit for requests of disclosure, etc.
Complete the required fields on the “Personal Information Disclosure Request” as they relate to the disclosure of personal information subject to disclosure, notification of purpose of use, revision, addition or deletion of personal information, suspension of use or erasure or personal information, and suspension of provision to third parties, etc., and submit it by postal mail to the Company’s contact desk for personal information.

(3) Fees related to requests for disclosure of information and notification of purpose of use
The Company levies a fee of 1,200 yen (consumption tax included) for each request of the disclosure of personal information and notification of purpose of use. When sending your “Personal Information Disclosure Request” or “Purpose of Use Notification Request” by postal mail, please enclose stamp duties or a postal money order for 1,200 yen.​ ​
*Postal money orders are available at a post office. The customer shall be responsible for any fees for the issuance of a postal money order.

(4) Persons who are able to request disclosure, etc.
Persons who can request disclosure, etc.:
① Individuals (the person whose personal information is subject to disclosure, etc.)
② Proxies (a person consigned by the individual or a legal proxy such as with parental authority) 

(5) Documents for confirmation of individual or proxy identity
Documents that confirm the identity of the individual making the request or the proxy are required when requesting disclosure, etc. (Power of attorney, etc.)
① If an individual is requesting disclosure, etc., one of the following must be provided:
・ Membership ID number and password
・ Copy of either driver’s license, health insurance card, or certificate of residence (issued within 3 months of the date)
・ Student ID or copy of student certificate
・ Copy of resident card
・ Proof of national certification​ ​

② If the person requesting disclosure, etc., is a proxy
One of the documents above and the power of attorney signed by the individual
Please also submit a seal of the individual and the seal registration certificate for the seal. If the proxy is a statutory proxy such as with parental authority, please submit proof of relationship with the individual.
If the proxy holds a certificate such as a lawyer, please note the occupation and their registration number.

2. Other
The Company may make changes to its Privacy Policy and procedures for disclosure, etc., without notice in order to in order to further improve its protection of personal information and address changes in laws, regulations and other rules.

Handling of Personal Information on Our Website
